Payment and Insurance Dr. Babula works with all insurance companies, either as an in-network or out-of-network provider. He also is an approved Medicare provider. When you first phone Dr. Babula's office, please have your insurance card handy so you can provide benefits information. You will be notified about coverage, deductibles, and co-payments prior to your first visit. As
a courtesy, Dr. Babula will submit all claims to insurance on your
behalf. If you have insurance coverage for the services, your
only responsibility is payment of any fees not covered
by your insurance carrier. If you do not have or want to use
insurance coverage, you may self-pay the entire fee.
